Skin grafts have a poor take rate in many circumstances including burns, and over the past 30 years, research aimed at improving these rates has been very active. The original state of the art was the split-thickness skin graft (STSG) where the epidermis and dermis layers are separated and the dermal component is reepithelialized.
Like the split-thickness skin graft, a split-thickness skin autograft (STSA) is the transfer of cutaneous tissue from one portion of the body to another. Unlike a split-thickness skin graft, the STSA is performed without separating the epidermis and dermis and incorporates the entire epidermis (including the dermis) in the area of healing. STSA is the most widely used form of skin graft in clinical practice and has shown comparable or better rates of graft take than split-thickness skin graft, especially in wounds with poor circulation, such as in the distal extremities and in acute wounds
